When submitting your request please include the name provided at the time of consignment and the mailing address in the "Message" space.

NOTE: Checks requested after 11/01/2023 will be available for in-store pick up only. Please include the preferred store pick up location with your request. We will notify you via email or phone call when your check is available and ready to be picked up. Thank you!
